The fundamental source of market failure is that individuals cannot judge the quality of their health care (e.g. Is Drug X really better than watchful waiting?). As a result, insurers are competing on the basis of quantity rather than quality, leading to excess provision and utilization rather than real improvements in quality. This is what is driving health care costs.
